Transform Health provides Non-Surgical Spinal Specialist services for NHS patients across Cornwall, for Cornwall Isles of Scilly Integrated Care Board (CIOSICB). We have been a commissioned NHS service since January 2023. That means you can receive our treatment as part of the NHS provision, as long as you are registered with a GP surgery within CIOSICB.
Your GP, First Contact Practitioner (FCP) or Extended Scope Practitioner (ESP) can refer you directly to our service. Alternatively, you can ask your physiotherapist, osteopath or chiropractor to request a referral through your GP.
If you have back +/- leg pain / sciatica that you have struggled with and not improved with treatment, your GP or First Contact Practitioner can refer you directly to us for help.
We deliver Cognitive Functional Therapy (CFT), which has good scientific evidence for improving pain and disability with long lasting effects. It is not giving you a sheet of exercises and letting you get on with it. Your therapist will work closely with you and take the time to listen to your problems in detail. We figure out what is causing your pain and help you to reduce its impact. We aim for you to get yourself better and stay better, with our guidance and support.
When we receive your referral, we will call you to discuss your issues to establish whether our service is most appropriate to you.
Our first consultation is a 30-minute telephone call where we discuss your issues and decide how to move forward. This process is essential to fully understand your symptoms, so that your treatment is best matched to your problems. You will be asked questions that you might not expect, to get to the bottom of the reasons for your pain.
If agreed, we will book a 90-minute appointment where all the issues around your spinal problems will be thoroughly explored. Appointments can either be by video call or in person and have found that video appointments can be as effective as face-to-face ones. Further treatment sessions will be agreed between you and your therapist. You can contact us at any time by phone, text or email to discuss any aspect of your care.
